- What to look for in postpartum underwear
Many new moms pack a coming-home outfit for their babies and for themselves, but they may forget one important item of clothing: postpartum underwear. The maternity underwear that you wore at the end of your pregnancy probably doesn't fit right anymore. And there are other important reasons to have comfortable postpartum underwear for what is commonly called your fourth trimester.
“After giving birth, our bodies are especially sensitive,” says Allison HillOpens a new window, an ob-gyn in Los Angeles. “The body’s healing process lasts about six weeks and often involves lochia, or postpartum bleeding, and discharge. Throughout this healing process, women need underwear that is comfortable, stretchy, supportive, and absorbent.”
You'll be wearing large pads to absorb the lochia (and maybe even the occasional urine leak, known as postpartum incontinence), and postpartum underwear should be able to hold them comfortably. You also don’t want your underwear to irritate any part of your body that's tender or swollen after a vaginal birth or c-section.
"The only postpartum underwear you need is something you can bleed on, pee on, and throw out. In other words, not La Perla,” says Dr. Lauren Streicher, clinical professor of obstetrics and gynecology at Northwestern University Feinberg School of Medicine and the medical director of the Northwestern Medicine Center for Sexual Medicine and Menopause. “Other than that, wear what is comfortable."
Here then, is what new moms in BabyCenter's Community and on our editorial team have found comfortable during the postpartum period.
How we chose the best postpartum underwear
- We talked to four moms on BabyCenter's editorial team who have four children between them, had both c-sections and vaginal births, and have worn many of the pairs on this list. They told us what postpartum underwear was comfortable for them immediately after birth and in the months afterward.
- We spoke with Dr. Hill about after-birth healing and what women should look for in postpartum underwear.

Best postpartum underwear overall
Fruit of the Loom Women's Cotton Brief Panty
These briefs from a trusted brand stretch and stay put, say moms who swear by the comfort of these underwear. Leah Rocketto, Associate Commerce Director for BabyCenter says that when she shopped for postpartum underwear, she wanted to find a pair that would be comfortable no matter how she delivered her baby.
"Fruit of the Loom briefs were roomy enough to accommodate the padsicles needed for vaginal birth recovery, and high-waisted enough so as not to irritate a c-section scar," says Leah, who ended up having a vaginal birth. Not only did the underwear hold her padsicles properly, but they held her body well, too. "I have a sort of large butt, and most underwear can ride up," says Leah. "But the Fruit of the Loom briefs provided plenty of rear coverage to prevent that from happening and causing additional discomfort in the postpartum phase."

Best postpartum disposable underwear
Frida Mom Disposable Postpartum Underwear
This is a better version of the disposable underwear you'll get in the hospital. This is largely because Frida Mom’s postpartum underwear is made of a moisture-wicking cotton that makes them soft and keeps them dry. "I lived in these for the first few days postpartum," says BabyCenter Senior SEO Content Strategist Sarah Darby, who adds that the hospital's underwear gave her a contact rash. “[Frida Mom’s] were way more comfy than the hospital ones, super stretchy and soft."

Some high-waisted briefs are intended to shape and smooth your body and are made of firmer fabric that holds you in, but not every mom wants that. If you're looking for affordable, absorbent, and disposable underwear, Always Discreet comes highly recommended by moms. They won't shift around and cause inadvertent leaks like pads sometimes do, especially when you're lying down.
Leah says a friend gave her a few packs of Always Discreet that she intended to use only in case of emergencies. But when her postpartum pads started shifting at night, she tried them. "They absorbed my discharge wonderfully, and were roomy enough that I could add a disposable ice pack and a few witch hazel pads for extra pain relief," says Leah. Best supportive postpartum underwear

Best mesh postpartum underwear

Some new moms are surprised to find how sweaty they are in the period immediately following birth as their bodies work to shed the water retained during pregnancy. Mesh panties are specially adapted for this purpose, made of light cloth that's full of tiny holes to let air circulate. In addition, mesh panties are supremely stretchy, and won't put extra pressure on any part of your body.
These mesh undies are breathable, disposable, and, unlike hospital panties, washable (moms say they'll go through the wash up to 10 times before falling apart). They're hardly lookers, but they're comfy and hold your pads, which is essential for many moms. And thanks to a high-and-wide waistband, they're unlikely to irritate either a sensitive and itchy postpartum belly or a healing c-section incision. Moms say they're more comfortable and durable than the mesh underwear nurses give you at the hospital, and they're easy to put on and take off even when it's difficult to bend over. Best of all, if anything leaks, you can throw these undies right in the trash, along with any pads you may be using.

What to look for in postpartum underwear
Your needs postpartum are largely going to depend on what type of labor and delivery you had — and not just in terms of vaginal vs. c-section. Even if you didn’t have a c-section, you may have to cope with an episiotomy incision or perineal tear, both of which can cause discomfort. You also have to make sure to find underwear that can adjust to your body, which will change shape and size as your uterus shrinks, and can fit pads to absorb the menstrual-like lochia that lasts up to eight weeks.
Because of these factors, you may consider the following things when shopping for postpartum underwear:
- Size: Postpartum underwear should be large enough to easily accommodate thick sanitary pads, especially in the first week when lochia is often heavier, Dr. Hill explains. Buy a size or two up from your pre-pregnancy size, to give you plenty of room and extra comfort.
- Price: Postpartum underwear generally aren't for long-term use, so you may want to economize.
- Fabric: Ideally, postpartum underwear will be made from a soft, breathable, and comfortable material like cotton, says Dr. Hill. Bamboo, modal, and other soft fabrics work for many women, too.
- Waistline: Women who have had c-sections will want high-waisted underwear, says Dr. Hill. “If the waistline isn’t high enough, the underwear elastic can cause irritation around the area, so I recommend looking for underwear that hits above the scar.”
- Special features: Some postpartum underwear have pockets meant to hold on ice packs, or panels made of silicone that won't irritate a c-section incision.
Types of postpartum underwear
Postpartum underwear come in three basic types:
- Disposable: If you give birth at a hospital or birthing center, you'll be given mesh underwear to wear after birth; the nurses may give you extra pairs to take home. They fit like boxer briefs and do a champion job of holding on pads. Some women prefer to wear underwear meant for incontinence instead of wearing pads to soak up the flow of lochia.
- Period or incontinence panties: Often paired with a pad for the first week or two, this type of underwear is thicker and designed to absorb period blood and/or urine. “Although it’s not always talked about, incontinence is common during postpartum recovery as the result of weakened muscles from birth,” says Dr. Hill.
- Compression: Compression garments fit tightly and are especially engineered to provide a medically beneficial level of pressure to the body. If you have questions, ask your doctor if a compression garment would help you. And while it may be tempting to opt for underwear with extra support — after all, the stomach can feel rather loose after childbirth as your uterus contracts — undergarments that are too tight can actually prevent the healing process, says Dr. Hill.
There are also plenty of pairs of underwear that are not sold especially for the postpartum period, but still work well for women. Some of these types include:
- High-waisted: With a waistband that rises above a healing c-section incision, these are often favorites of moms who gave birth via c-section. They're usually less expensive than special postpartum underwear, too.
- Briefs: With a higher waistband, full rear coverage, and a wider crotch area than bikini underwear, high-cut underwear, or thongs, these undies are good for the weeks when new moms wear larger pads.
- Boxer briefs: Originally marketed to men, these underwear are shaped like boxers but are stretchy like briefs. They're large enough to hold on pads, and generally have a higher waistband that won't irritate incisions.
- Boy shorts: These underwear hold on pads particularly well, but make sure that the waistband is high enough to avoid a c-section incision, if you have one.
- Seamless: Knitted as one piece, seamless types of underwear may be less irritating if you have a sore or swollen vaginal or perineal area after birth, or any incisions.
How many pairs of postpartum underwear do I need?
It depends on how often you do laundry, but consider that you may have to change your underwear more often during the postpartum period. At least seven to10 pairs should get you through a week without doing a load of wash.
